Care Coordination

What is "Care Coordination"?
Care Coordination is a project of Communities In Schools-Central Texas, Inc. This project is funded and managed by the Travis County YFAC (Youth and Family Assessment Center). Care Coordination involves families from identified AISD schools in the wraparound approach to service delivery.

What is wraparound?
Wraparound is a strengths-based model of social service delivery that offers the entire family support to meet their goals. A Child and Family Team is created by the parent/primary caregiver and the Care Coordinator, which typically includes a school representative, service providers, family members, a neighbor or friend, and others. This team meets regularly to develop and implement a Plan Of Care, which defines the family's needs, strengths, goals, and planned interventions. The team can use a network of service providers, including therapists, mentors, parent coaches, case managers, and others to assist the family.

Who is eligible?
Families with a child attending one of the projects identified Austin ISD schools:

  • Allison Elementary School
  • Oak Springs Elementary School
  • Andrews Elementary School
  • Harris Elementary School
  • Zavala Elementary School
  • Ortega Elementary School
  • Martin Middle School
  • Mendez Middle School
  • Webb Middle School
  • Kealing Middle School
  • Dobie Middle School
  • Rodriguez Elementary School

In addition, the identified child/family must meet the following criteria:

  • have needs in multiple areas, such as school success, mental health, family relationships
  • be willing and able to actively partner with our project to help ensure success
  • not currently be on juvenile probation
  • be low income

 

What is the cost?
There is no cost to the family or the school.

How long does it take?
Once the family is recommended for Care Coordination, the assigned Care Coordinator contacts the family and, if the family meets eligibility and agrees to enroll, the Care Coordinator will start working with the family as soon as there is capacity on their caseload. The length of service in our project varies depending on the family's needs, but on average families are enrolled in Care Coordination for about a year.

How can a family be recommended for Care Coordination?
The school's IMPACT team refers the identified child and their family to our project. A representative of the IMPACT team should discuss Care Coordination with the family and complete a Care Coordination Referral Form with the family prior to making the referral. Anyone who identifies a child and family who would benefit from Care Coordination can ask the IMPACT team to consider referring the family to our project.
